[Localized prostate cancer: effectiveness and safety of treatment options] Quebec: Institut national d'excellence en sante et en services sociaux (INESSS). 2016 Authors' conclusions In the published literature, often consisting of studies of poor methodological quality, no treatment modality was generally shown to be superior over the others in terms of effectiveness and safety. The different treatment options are equivalent with respect to prostate cancer mortality and all-cause mortality. Adverse effects nevertheless vary with the treatment option. Prostatectomy may greatly affect short-term quality of life and may cause incontinence and significant erectile dysfunction. Radiation treatments may cause urinary tract irritation and obstruction as well as sexual and gastrointestinal function problems. Surveillance, while suspected to cause anxiety in patients, has not been associated with major adverse effects.
Given the risks and benefits associated with each of these options, medical staff should explain the different treatments to the patients in order to help them to make a choice corresponding in order to make a choice corresponding to their values and preferences. This decision process could be facilitated by the presence of an interdisciplinary committee and the use of decision-support tools.
