[Home-based rehabilitation of patients suffering from stroke] Copenhagen: Danish Centre for Evaluation and Health Technology Assessment (DACEHTA). 5(1). 2005 Authors' objectives Home-based rehabilitation has for a number of years been researched as a promising alternative to conventional rehabilitation of persons suffering from stroke in hospital care. Annually more than 5000 stroke patients need rehabilitation in Denmark. The present report has focus on rehabilitation from the time of medical stabilization after admission to the termination of medical rehabilitation passing the further responsibility for the patient to the municipal authorities. Authors' conclusions Despite positive (socio) economic as well as qualitative effects of EHSD practical constraints do exist. In practice the implementation of domiciliary rehabilitation is expected to be halted by a conflict of financial interests between the hospitals and the municipalities. This is due to the fact that the hospitals which organize the home-based rehabilitation will be left with a financial burden, whereas the municipalities without using any additional resources will save care expenses. To overcome this constraint a flexible model is outlined, where the hospitals are partially compensated for the expenses towards outgoing therapists. In Denmark, this financial barrier is supposed to be minimized through an administrative healthcare reform, which is expected to be implemented from 2007.
