AETSA 2011 /1. 2013 Authors' objectives To assess CT colonography accuracy, security and efficiency in comparison to colonoscopy for CRC screening. Authors' conclusions CT colonography pooled sensitivity for CRC screening was lower than for colonoscopy, both per lesion or per patient analysis. Only in a specific subgroup with polyps between 8 and 10 mm and according to analysis per lesion CT colonography showed higher pooled sensibility than colonoscopy. The pooled specificity of CT colonography was higher than that colonoscopy according to analysis per patient, but not in analysis per lesion. .Accuracy get higher according to lesion size with maximum for lesions >10 mm. Adverse effects incidence for CT colonography and colonoscopy was low. None study analyzed the long-term side effects derived from the total CT colonography radiation emitted. Interpretation of these results could be limited by bias risk detected in included studies (lack of blinding or adequate gold standard).
The evidence for CTC efficiency in comparison to no screen was favourable for CTC screening, however, in comparison to others screening tests (FOBT and colonoscopy) varied in terms of cost/utility and cost/effectivity. Studies with direct comparisons were needed in our environment.
