Healthcare Technology Brief Publication. 2014 Authors' conclusions Ventilator-associated pneumonia (VAP) is a common healthcare-associated infection in the United States, and accounts for half of all cases of hospital-acquired pneumonia. VAP is defined as pneumonia arising within 48 to 72 hours after insertion of an endotracheal tube (ETT) and initiation of mechanical ventilation, and is characterized by the presence of a new or progressive infiltrate on radiology, signs of systemic infection (fever, leukocytosis), changes in sputum characteristics (purulent tracheobronchial secretions), and detection of the causative agent. While the true incidence of VAP is difficult to determine as its definition is nonspecific and subjective, for the year 2012, National Healthcare Safety Network (NHSN) facilities reported approximately 4000 VAPs, and the incidence among various hospital units ranged from 0.0 to 4.4 per 1000 ventilator days.
Approximately 5% to 15% of ventilated patients develop VAP, and the attributable mortality is 10%. Adjuncts to usual care have been evaluated to reduce the bacterial colonization of ETTs with the goal of reducing the incidence of VAP.
