Healthcare Technology Brief Publication. 2014 Authors' conclusions Many surgeries are associated with uncontrolled bleeding and excessive blood loss, either as a natural consequence of the anatomy involved, an underlying coagulopathy, or due to complications during surgery. Uncontrolled perioperative bleeding can reduce surgical field visibility and prolong surgery. Excessive blood loss causes fluid shifts that can cause coagulopathy. The primary treatment for excessive blood loss, transfusion of blood products, carries its own risks, and increases perioperative morbidity and healthcare costs. Certain complex surgeries such as spinal and vascular procedures require the disruption and manipulation of highly vascularized areas. Coronary artery bypass grafting (CABG) necessitates the use of cardiopulmonary bypass (CPB) is associated with high rates of bleeding. Other situations associated with difficult-to-control bleeding that may not respond to standard measures (e.g., electrocautery and suture ligation) include the presence of friable tissue, postoperative adhesions, diffuse soft tissue bleeding, and bone bleeding. Adjunctive topical hemostatic agents are useful when bleeding at the surgical site is difficult to control by traditional techniques (suturing, cautery, manual compression) or when bleeding occurs in areas that are difficult to access. These agents can halt the cascade of hemostatic abnormalities at the bleeding source during surgery that lead to hemorrhage and need for transfusion or other systemic approaches.
